Terms of Reference
- Frequency: Pharmacovigilance and Therapeutic review meetings are being conducted once in 3 months.
- Quorum: 50% of membership.
- Meeting venue: As decided by the Chairperson.
Responsibilities
- To formulate and implement the policies and procedures relating to pharmacy services and medication management process.
- To formulate and implement the hospital formulary and revise the same as per the policy.
- To evaluate New drug request, Non formulary drug requisition, etc in the committee.
- To define and analyze adverse drug reactions and patient safety incidents involving medications (medication errors, etc).
- To design and implement methods for ensuring the safe prescribing, distribution, administration and monitoring of medication.
- To conduct training programmes related to medication safety.
